    Forms support placeholders like {{practice_name}}, {{practitioner_name}}, and {{client_name}}. They resolve automatically at send time using your practice profile and the contact you are sending to — no copy-paste, no typos in client-facing copy.

  • One-click email dispatch with secure links

    Send an intake or consent form to a contact in one click. Each form link is an HMAC-signed token that expires after 24 hours, scoped to that contact and that template. No client login or app install required to fill it out.

  • Submissions auto-create or update contacts

    When a new client submits an intake form via your public profile, a contact record is created automatically with the email verified. Returning client submissions update the existing contact and append to their form history — no manual data entry.

  • Branded PDF export with practice letterhead

    Every submission generates a PDF stamped with your practice name, address, ABN, and logo. PDFs are stored against the contact and downloadable from the form history panel — ready for clinical records or referrer requests.

  • Consent capture with revoke audit trail

    Consent-type forms record consent_granted and a digital signature. Revoking consent later is a soft revoke — the original submission, signature, and revoke timestamp are preserved for compliance. Forms are never silently deleted.

  • Form versioning that never breaks history

    Editing a template schema bumps its version automatically. Past submissions stay tied to the version they were filled against, so historical PDFs render exactly as the client saw them. Renaming a form does not bump version — only schema changes do.

  • Public online booking with email OTP

    Clients book directly on your public profile with a 6-digit email one-time-password — no client account, no app download. OTP verification uses cryptographically secure randomInt and timing-safe comparison. Returning clients can use OTP login to self-serve rebookings and view their submission history.

  • Configurable booking rules per practitioner

    Set business hours, slot duration, minimum advance notice (default 12 hours), and maximum future bookings per client. Rules cascade from per-practitioner override to system defaults — admins tune defaults without redeploying.

  • HIPAA-aligned by default

    All form submissions, signatures, and PDFs are stored with the same field-level AES-256-GCM encryption and row-level access control as session notes. Form access is team-scoped — practitioners see only their own practice data.

  • Free plan includes intake forms and booking

    The free plan covers up to 20 active contacts, intake forms, and online booking with no credit card. AI transcription and note generation unlock on paid tiers — forms and booking are not gated.

New client onboarding

Send your intake form the moment a booking is confirmed. The client fills it out before the first session, the contact is created with verified email, and a branded PDF lands in their record — all without you touching the keyboard.

Recurring consent renewals

Use the consent form type to capture clinical consent with signature and timestamp. When consent needs to be revisited, send the form again — past versions and revocations are preserved as an immutable audit trail.

Outcome measurement between sessions

Email a PHQ-9 or custom assessment form between appointments. Submissions append to the contact's form history so you can review trends over time alongside your session notes.
